Pronounced DOA in Chicago. Cause of death determined to be respiratory failure.

Note 22Physical Exam: Satisfactory. All vaccines and antibiotic shots were given with no adverse short-term effects. Blood tests revealed slight risk for CPAID. Overall range of motion measured to be slightly greater than normal. Displayed extreme aversion (marked physical and emotional distress) to separation from presumed sibling (F003).

Diff. Characteristics: Reddish birthmark on the right shoulder blade. Scarring on the inside of left leg.

Cog. Abilities: Results on HMT, GCCB, and DCA were standard for age subgroup. No special capabilities detected.

Prelim Phys. Assmt.: Displayed no special physical abilities or abnormal difficulties in learning preliminary skills. Showed no particular talent for performance but not reluctant to learn skills.

Physical Exam: Satisfactory. All vaccines and antibiotic shots were given. Mild rash detected 12 hours following shots. Blood tests revealed preliminary signs of CPAID. Overall range of motion measured to be greater than normal. High level of flexibility. Displayed aversion to separation from presumed sibling (F002). Emotional and physical distress less marked than F002.

Diff. Characteristics: N/A

RD 9.4Cog. Abilities: Results on HMT and GCCB were standard for age subgroup. DCA results showed above average creative capacities. Creative problems solving development should be monitored.

Prelim Phys. Assmt.: Displayed no special physical abilities or abnormal difficulties in learning preliminary skills. Showed no particular talent or interest in performance.

***CORRECTION: Displayed great physical prowess and performance ability. Should be arena ready following 30 day prelim period.***

Physical Exam: Satisfactory. All vaccines and antibiotic shots were given with no adverse short-term physical effects. Displayed aversion (marked emotional distress) to medical tests. Sedation and restraining straps recommended for use in future exams. Blood tests normal. Overall range of motion measured to be normal.

Diff. Characteristics: N/A

RD 9.5Cog. Abilities: Results on HMT, GCCB, and DCA were all significantly above average. Cognitive and creative development must be monitored closely.

***NOTE: Displays the ability to influence the behavior of others. If possible, should be moved prior to 30 day prelim period.***

Cog. Abilities: Results on HMT, GCCB, and DCA were standard for age subgroup. No special capabilities detected.

Prelim Phys. Assmt.: No assessment available at this time.

***NOTE: Suffered severe asthma attack following routine showering. Required two days of monitoring. Will need to reassess following 30 day prelim period.***
